Data Privacy Request

Request access, correction, deletion, a change to communication preferences or review of another privacy concern. Rights and exceptions depend on applicable law; submitting a request does not establish a statutory entitlement.

Requests are stored privately and a staff notification is attempted to info@vialrelay.com. Identity and any representative’s authority must be reasonably verified before information is disclosed or changed. This form does not automatically access, correct or delete any record.

No identity documents or patient information are needed to begin. If further verification is appropriate, staff must arrange an authorized process and explain what is necessary.
